#Inspire

Signposting Form

The Disability Sport Wales (DSW) #inspire campaign is a way that you, or someone who knows you, can let us know that you have the potential to achieve more in para and/or disability sport.

To make us aware of this there is certain information we need to collect from you now (the information on this form) and DSW collects this as part of entering into a contract to support you. There may be information we need from you in the future, and we may need your consent for some of this information – but we will ask you for that at the time we need it. If you do have to ask for more information we will let you know what we need, why we need it, what we will do with it, and how long we will keep it.

We will use the information on this form to:

  • Help us understand your potential and which sports you have experience of already.
  • Get in touch with you to talk to you about what you would like to be able to achieve in sport.

We might share your information with other people in DSW (perhaps a DSW Development Officer, the CEO, and Administration Officer, etc) or with an NGB (National Governing Body) for a sport you could be involved with. Where we do share information we will make sure that we do it safely and the people who receive it look after it and also keep it safe.

You can tell us to stop using, or sharing your information at any time, but this may stop us being able to support you to the best of our abilities. If you want us to stop using your information please contact: inspire@disabilitysportwales.com.

DSW will keep your information for the time you are involved with us, and for 12 months after that. If you want to get more information about what all this means please contact the DSW Performance Pathway team.
